This Privacy Policy explains how we collect, hold, use, disclose and protect personal information, and how patients may access or correct their information or make a privacy-related complaint.
We collect personal and health information primarily to provide and coordinate healthcare. Information may also be used for directly related practice activities, including Medicare and other financial claims, billing, accreditation, quality improvement, clinical audit, staff training and lawful business operations.
When you register with our practice and receive healthcare, your information is accessed only by team members who require it for authorised duties. Additional consent will be obtained where required if information is to be used or disclosed for another purpose, unless otherwise permitted or required by law.

Information is usually collected directly from you during registration, consultations, telephone calls, email or SMS communication, online bookings and other interactions with the practice.
Where necessary for your healthcare, information may also be obtained from a guardian, carer or responsible person; other healthcare providers such as specialists, allied health professionals, hospitals, pathology and diagnostic imaging services; Medicare, health funds, the Department of Veterans’ Affairs or other authorised agencies. Information may also be collected or exchanged through approved electronic health systems, including electronic prescriptions and My Health Record, where applicable.
You may deal with the practice anonymously or using a pseudonym where this is lawful and practicable. Identification may be required where necessary for safe clinical care, Medicare claiming, prescribing, referrals, healthcare identifiers or other legal and operational requirements.
Personal information is used and disclosed only for authorised purposes. This may include sharing relevant information with other healthcare providers involved in your care, authorised service providers to the practice, approved electronic health systems, or where disclosure is required or authorised by law.
Except where permitted or required by law, we will not disclose your personal information to a third party without appropriate authority or consent. Only information relevant to the authorised purpose will be disclosed.
We do not routinely disclose patient information to overseas recipients. If an overseas disclosure is contemplated, applicable privacy requirements will be considered and patients will be informed or consent obtained where required.
Health information will not be used for direct marketing unless the patient has provided the consent required by law. A patient who has consented may withdraw that consent.
Patient information may be used for quality improvement, accreditation, audit or other activities where permitted. Where information is used or provided in de-identified form, reasonable steps are taken so individuals are not reasonably identifiable. Research involving identifiable health information will be managed in accordance with applicable consent, ethical and legal requirements.
Patient health records are maintained primarily in our electronic clinical information system. We use administrative, physical and technical safeguards to protect personal information from unauthorised access, loss, misuse or disclosure.
Access to patient information is restricted to authorised team members who require it for legitimate duties. Employees, contractors and other authorised persons are required to maintain confidentiality. Paper or temporary records containing confidential information are stored securely and disposed of through approved confidential-waste processes when no longer required.
Health records are retained for at least the minimum period required by applicable Queensland law and other legal or professional obligations. Records are securely destroyed or de-identified when they are no longer required to be retained.
You may request access to, or correction of, personal information held by the practice. Requests may be made in writing or by contacting us. We may require reasonable identity verification before information is released.
We will respond to access and correction requests within the timeframes required by applicable privacy law. If access or correction cannot be provided, you will be informed as required by law.
We take actual or suspected privacy breaches seriously. Incidents are assessed, contained and documented, and notifications are made where required under applicable privacy or digital-health legislation.
